From the physiology of gastric emptying to the understanding of gastroparesis

چکیده
The stomach is the segment of the gastrointestinal tract in which the main secretory functions and digestion in the alimentary canal begins. However, it’s most critical and important function in digestive physiology is gastric motility (GM). Normal GM is controlled by diverse extrinsic and intrinsic stimuli. The main extrinsic regulation comes from the vagal nerve. Although the majority of vagal efferents that arrive at the stomach are excitatory, some vagal nerve endings are inhibitory. They produce stimuli through neurotransmitters such as nitric oxide and vasoactive intestinal peptide (VIP). Intrinsic stimuli come from the enteric nervous system and are important in the coordination of gastric motility with more distal segments of the digestive tract especially during the interdigestive period (Figure 1). These myenteric neurons also communicate with the different pacemaker cells of the digestive tract. They are also known as interstitial cells of Cajal (ICCs) and are located in the circular muscle layer of the stomach and the proximal intestine. GM has three basic components. Even though each one of these phenomena has independent moments and mechanisms, normal gastric emptying is only achieved when there is a functional integration of the three activities (1-4).

Efficacy of prokinetic agents in diabetic gastroparesis comparing symptomatology and scintigraphy – An open-label trial
Background: It is pertinent to objectively assess the severity of diabetic gastroparesis and tailor treatment accordingly. The current study was planned to document gastroparesis by gastric emptying scintigraphy (GES) objectively and see the effect of medications and diet control on clinical and GES after four weeks. Methods: A prospective, open-label randomized trial was conducted in the Depa...

Objective(s): Effects of ethyl acetate extract of Salsola collina (EES) on brain-gut peptides and interstitial cells of gastric Cajal in rats with diabetic gastroparesis were explored. Materials and Methods: Rats were divided into six groups: normal control group (NC), diabetic gastroparesis model group (DGP), low, medium, and high dose ...
متن کاملSodium hydrosulfide: A new potential candidate for treating delayed gastric emptying in diabetes
Introduction: Sodium hydrosulfide (NaHS) has shown to enhance the gastric emptying rate in normal rats but till now its effect on gastric emptying of food stuffs in diabetic rats was not investigated. Therefore, this study designed to determine the role of an oral administration of NaHS on gastric emptying rate (GER) of glucose, albumin and olive oil in gastroparetic and normal rats. Methods: T...

Gastroparesis is a disorder with a lack of treatment options and this study investigated the effect of electroacupuncture on gastric emptying and involved mechanisms. Dogs implanted with a duodenal cannula and serosal electrodes were fed with Ensure mixed with phenol red, and the gastric effluent was collected. Electroacupuncture was performed from 30 min before until 45 min after the meal. Gas...

Objective(s):This study was carried out to investigate the effects of COX-2 selective inhibitor (Celecoxib) or non-selective COX inhibitor (Ibuprofen) on gastrointestinal motility. Materials and Methods: The rats were randomly divided into five groups including: intact, sham, traumatic brain injury (TBI) group (intact rats under TBI), Celecoxib group (10 mg/kg), Ibuprofen group (10 mg/kg). متن کاملSynchronized gastric electrical stimulation improves delayed gastric emptying in nonobese mice with diabetic gastroparesis.
The aim of this study was to investigate the effect and mechanism of synchronized gastric electrical stimulation (SGES) on gastric emptying in nonobese mice with diabetic gastroparesis (DB-GP). Eight control mice and 48 nonobese diabetic (NOD) mice with two pairs of gastric electrodes were used in this study. The study included seven groups in a randomized order [control, diabetes (DB), DB-GP, ...
